First Visit

Your initial appointment will consist of a codiagnostic examination and explanation of any concerns. We will review your medical history to make sure any treatment you may require is safe and appropriate for you. If you do not have current x-rays we will take a complete series to evaluate those areas that are not evaluated by visible examination. The information we gather will allow us to schedule a consultation and review our findings and recommendations for treatment. We will schedule most treatment only after you have reviewed your options.

Please assist us by providing the following information at the time of your first visit:

  • X-rays (if applicable)
  • A list of your current medications
  • If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the consultation visit.

X-Rays

If you have x-rays from a previous dentist or physician, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility as needed.
